What Is Back Pain?

Back pain is mechanical dysfunction of the spinal joints, discs, muscles, or nerves of the back, most commonly in the lumbar (lower) region. It is one of the most common health complaints worldwide, affecting up to 80% of adults at some point in their lives, and ranges from a dull, persistent ache to a sudden, sharp sensation that makes movement impossible. The lumbar spine (lower back) bears the majority of the body's weight and is the most frequently affected region, though mid-back and neck pain are also widespread. In Malaysia, back pain is among the leading causes of workplace absenteeism and reduced quality of life. Despite how common it is, back pain is rarely addressed at its mechanical root — most people cycle through painkillers, heat packs, and rest without resolving why the pain keeps returning. The Gonstead method works differently: it identifies which specific spinal segment is dysfunctional and corrects only that, allowing the whole spine to function normally again.

Root Causes

What Causes Back Pain?

Back pain is almost always mechanical — something in the spinal structure is not functioning the way it should. Identifying the source is what separates temporary relief from lasting recovery.

Muscle Strain & Ligament Sprain

Sudden awkward movements, heavy lifting, or repetitive strain can overstretch muscles and ligaments in the back. This is the most common cause of acute back pain, often felt as a sudden 'catch' in the lower back.

Poor Posture & Prolonged Sitting

Sustained forward-flexed postures place uneven load on spinal discs and facet joints. Years of desk work, driving, or looking down at phones accelerates spinal wear and shifts vertebral alignment.

Disc Degeneration & Bulging

Worn or herniated discs reduce the cushioning between vertebrae and can compress nearby nerves. Disc problems are responsible for a significant proportion of persistent, radiating back pain cases.

Facet Joint Dysfunction

The small facet joints linking each vertebra can become inflamed or misaligned, causing localised pain that worsens with twisting or bending backward. Often felt as stiffness after rest.

Spinal Subluxation

Vertebrae that shift out of optimal alignment alter how load is distributed across the spine, creating chronic stress points and irritating nearby nerves. This is the primary focus of Gonstead assessment.

Sedentary Lifestyle & Weak Core

Weak abdominal and back muscles provide insufficient support for the spine. Without adequate muscular support, the vertebral structures absorb forces they are not designed to handle alone.

Progression

How Back Pain Progresses

Back pain that isn't treated at its source tends to worsen over time. Understanding the stages explains why early precision care produces far better long-term outcomes than waiting it out.

Stage 1Mild

Acute Pain (0–6 Weeks)

Sudden onset, often after a specific incident such as lifting or twisting. Pain is sharp or intense and may limit movement significantly. Inflammation is high. Early chiropractic care at this stage is highly effective and can prevent the condition from progressing to chronic.

Stage 2Moderate

Subacute Pain (6–12 Weeks)

Pain fluctuates but persists beyond the normal healing window. The body begins adapting to the dysfunction — developing compensatory muscle spasms and altered movement patterns that can create secondary pain elsewhere. The underlying cause remains unresolved.

Stage 3Severe

Chronic Pain (3+ Months)

Persistent pain that has become part of daily life. Structural changes — disc degeneration, facet joint wear, postural compensation — may now be visible on imaging. Sleep quality, mood, and productivity are frequently affected. Recovery is possible but takes longer.

Stage 4Critical

Degenerative Cascade

Long-term unresolved spinal dysfunction accelerates disc degeneration, osteophyte (bone spur) formation, and spinal stenosis. This stage represents irreversible structural change. While full reversal is no longer possible, proper chiropractic care can still significantly reduce pain and slow further deterioration.

Chronic back pain is not inevitable.
