Sleep Positions When You're Pregnant: What Helps and What to Avoid

From around week 20, the way you sleep starts to matter more than at any other point in your life. There's a reason most prenatal advice converges on the same answer — left side — and it's not arbitrary. Anatomy, blood flow, and structural load all push in the same direction.

Why Side Beats Back After 20 Weeks

By the second trimester, the uterus is heavy enough that lying flat on the back puts compression on the inferior vena cava — the major vein that carries blood from the lower body back to the heart. When that vein is compressed, blood return slows, blood pressure can drop, and placental blood flow can reduce. Many women feel it as light-headedness or breathlessness within a few minutes of lying supine. Beyond the discomfort, sustained compression can affect the oxygen the baby receives. From around 20 weeks, side-sleeping is recommended for this reason — not because back-sleeping is dangerous in short bursts, but because hours of it overnight isn't ideal.

Left Side vs Right Side

The left side gets the slight edge in most prenatal guidance. Anatomically, the inferior vena cava sits to the right of the spine, so lying on your left side keeps the weight of the uterus off it. Left-side sleeping is also associated with slightly better placental blood flow in some studies. That said — and this matters — right-side sleeping is fine when the left isn't comfortable. Switching during the night is normal and welcome. The aim isn't perfection on one side all night; it's avoiding sustained back-sleeping. Mostly-left, occasional-right is a perfectly reasonable target.

The Pillow Setup That Actually Works

  • A pillow between the knees — keeps the pelvis level and prevents the top knee from dragging the upper hip forward, which strains the SI joint. This single change is often the most relief-producing one.
  • A small pillow under the bump — once you're large enough that the bump pulls down on your spine when you're side-lying, a thin pillow underneath supports the weight and reduces lumbar strain.
  • A body pillow (optional) — particularly useful in the third trimester for women who find themselves rolling onto their back. A C-shaped or U-shaped body pillow makes back-sleeping physically difficult and side-sleeping comfortable.
  • A pillow under the upper arm — small detail, big difference for shoulder pain. Without it, the upper arm hangs forward and pulls on the shoulder joint for hours.

How to Retrain a Stomach or Back Sleeper

  1. Use the body-pillow wall trick — place a long pillow against your back (or front, for stomach sleepers). It physically blocks rolling and gives the body a cue that there's a boundary on that side.
  2. Build the habit before the bump grows — if you're early in pregnancy, start practising side-sleeping now. Once the bump is larger, retraining is harder because comfort is harder to achieve from any angle.
  3. Use gradual angle changes — for back sleepers, start by propping yourself at a 45-degree angle with pillows, then progress to fuller side-lying over a few weeks. A sudden switch tends to fail; a gradual one tends to stick.

When Sleep Positioning Isn't Enough

Sometimes the position is right, the pillows are right, and sleep is still painful. That's usually a sign there's a structural component — pelvic misalignment, SI joint irritation, or lumbar shift — that positioning alone can't fix. The Webster technique within Gonstead chiropractic is a gentle, side-lying approach designed for pregnancy that addresses pelvic alignment without any twisting or pressure on the abdomen. In our clinical experience, many patients report that sleep becomes meaningfully easier within a few sessions, simply because the joint that was hurting all night is no longer mis-loaded. If sleep is the main issue and you've already optimised the position, it's worth being assessed. We see pregnant patients across Sunway Geo, Sri Petaling, and Kota Damansara. WhatsApp the branch closest to you to book.
