The Cracking Sound: What It Actually Is (and Isn't)

The cracking sound that sometimes accompanies a chiropractic adjustment is harmless, well-understood, and not actually the point of the adjustment. It's a phenomenon called cavitation — a small, momentary change in pressure inside the joint capsule — and it has nothing to do with bones being "put back into place". A successful adjustment may produce a sound, or it may not. Either way, what matters is whether the joint moved through the intended range, not whether it was audible while doing so.

What the Sound Actually Is

Synovial joints — the kind that make up most of the spine — are filled with synovial fluid, a slightly viscous lubricant containing dissolved gases like nitrogen, oxygen, and carbon dioxide. When the joint is moved through a quick range that briefly increases the volume of the joint capsule, the pressure inside drops. That drop in pressure causes some of the dissolved gas to come out of solution as a small bubble — a process called tribonucleation. The bubble forms, and depending on the dynamics, may collapse or stabilise. The "pop" is the sound of that gas-bubble formation. Recent MRI studies have actually captured this happening in real time, settling decades of debate about exactly what the sound was. It's the same physics that makes your knuckles crack when you pull on your fingers, the same physics that occasionally makes your knees pop when you stand up. The joint isn't damaged, dislocated, or "reset" — it's just gas behaving like gas.

Why It's Harmless

The myth that cracking joints causes arthritis has been studied repeatedly and consistently debunked. The most famous study on this is probably Donald Unger's — a doctor who cracked the knuckles on only one hand, every day, for over sixty years, and found no difference in arthritis between his two hands. (He won an Ig Nobel Prize for it, and the underlying point has been confirmed by larger population studies since.) Long-term studies of habitual knuckle-crackers don't show elevated rates of joint disease, structural damage, or reduced grip strength. Cavitation isn't a stress event the way a sprain or impact is — it's a fluid-dynamic phenomenon happening inside a sealed capsule. Spinal adjustments delivered with appropriate technique fall squarely within the safe range of joint motion. The audible release is incidental to the process, not destructive of it.

Why Some Adjustments Don't Make a Sound

Not every successful adjustment produces an audible release. Sometimes the joint geometry simply doesn't generate cavitation at the angle and force used. Sometimes there's already gas in the joint from recent movement and the bubble doesn't form again right away — there's a refractory period of about 20 minutes after a cavitation before the same joint can produce another. Sometimes the patient is tense and the joint doesn't move through the full range we were aiming for, even though the contact and force were correct. A silent adjustment can be just as effective as a noisy one. The test of effectiveness isn't sound — it's whether the segment moved through its intended range, whether the post-adjustment palpation findings change, and whether the patient experiences lasting symptomatic and functional improvement. Patients sometimes worry that "no crack" means "no fix". It doesn't.

What the Sound Doesn't Mean

A few persistent myths worth clearing up. The sound is not "putting bones back into place" — vertebrae do not pop in and out of joint, and they don't subluxate in the orthopaedic sense (full dislocation) or you'd be in an emergency room, not a chiropractic clinic. The sound also isn't an indicator of force level. Patients sometimes assume a louder pop means a harder adjustment. It doesn't — cavitation volume is mostly a function of joint volume and gas dynamics, not the amount of force applied. A specific Gonstead adjustment can be relatively quiet and still produce excellent results, while a generalised manipulation can be very loud and still leave the actual problem segment untouched. The number of pops also isn't a measure of effectiveness. We see patients who come in expecting a "good crack" and feel mildly disappointed if they only hear one — but a single specific cavitation at the right level is, almost by definition, exactly what we want. More sounds across more segments often mean we've moved healthy joints too, which isn't the goal.

The Gonstead Approach to Specificity

Gonstead adjustments are designed around precision, not noise. The whole point of the five-point analysis — visualisation, instrumentation, static palpation, motion palpation, and X-ray — is to identify the single specific segment that needs movement, so we can deliver one focused adjustment to that level rather than a generalised crack across the whole region. With this approach, the goal of the adjustment is a very specific motion of one vertebra in a specific direction. Whether that motion happens to produce a cavitation sound is a secondary outcome. We sometimes describe it to patients this way: the question we're asking after an adjustment isn't "did it crack?", it's "did the segment move the way it was supposed to, and is the post-adjustment exam different?". For our chiropractors, the post-adjustment palpation — re-checking tissue tension, motion, and Nervoscope reading at the same level — is the real feedback loop. Sound or no sound, that's how we know whether the adjustment landed.

When You Should Be Concerned About a Sound

Most cracking sounds, whether self-induced or during an adjustment, are harmless. There are a few signs that warrant attention. Pain that occurs at the same moment as the sound — not the brief pressure of the adjustment, but a sharp pain — is worth flagging. Swelling that develops afterward in the joint area is unusual and should be evaluated. Pain that persists more than 24 to 48 hours after the adjustment, rather than easing, is a sign to come back for review. Any new neurological symptoms — numbness, tingling, weakness — that weren't there before the adjustment warrant prompt re-evaluation. None of these are common after a properly delivered adjustment, but they are reasons to call us rather than wait it out. The same applies to your own habit of cracking your back or neck — if it's becoming painful or the sounds are accompanied by symptoms, get it checked.

FAQ

Is cracking my own back bad?

Generally not, but it's often unhelpful. Self-cracking tends to mobilise the segments that are already hypermobile — because those are the easy ones to move — while leaving the genuinely stuck segments untouched. Over time this can reinforce the imbalance rather than correct it.

Why do I crack so easily?

If your spine cracks with very small movements throughout the day, the involved segments are likely hypermobile. That's a stability issue, not a flexibility win. A specific assessment can identify which levels are unstable and what to do about them — often involving strengthening rather than more cracking.

Should I be worried if my chiropractor doesn't make my back crack?

No. A silent adjustment is a normal, valid outcome. If the chiropractor's post-adjustment exam shows the segment moved through its intended range, the adjustment did its job whether or not there was a sound.

Why does it feel different in different parts of the spine?

Different regions of the spine — cervical, thoracic, lumbar — have different joint geometry and different surrounding muscle bulk, which changes both how the adjustment is delivered and how the sound transmits to your ear. Neck adjustments often sound louder simply because your ears are closer to the joint.

Can I get the same effect from cracking myself?

No. The sound is the same physics, but a self-induced crack rarely targets the segment that actually needs movement, and the angle and force you can apply to your own spine are limited. The therapeutic effect of a chiropractic adjustment comes from specificity, not from cavitation.
