Hip pain when running is rarely just "the hip". It's almost always a chain — feet, knees, pelvis, lower back — and identifying the weak link is the whole game. Patients come in convinced they have a hip problem, and roughly half the time the hip is just the loudest voice in a quieter conversation happening upstream. Here's how to read the signs.
In our clinical experience, hip pain in runners falls into one of six categories:
A quick self-triage:
This is a starting point, not a diagnosis. Two causes can coexist — IT band irritation often masks an underlying glute medius weakness, for example.
Sacroiliac joint dysfunction is the cause that gets misdiagnosed most often. Patients are told it's "just" hip pain or "just" lower-back pain, when the actual issue is the joint where the two meet. The pelvis is the platform for hip motion. If one side sits higher or rotates further forward than the other, every running step delivers an asymmetric load to the hip joint above and the knee below.
We have seen runners chase IT band tightness for months when the upstream fix was a pelvic adjustment plus glute work. The IT band tightens because the pelvis isn't level, not the other way around.
Most non-acute hip pain responds to a few weeks of disciplined home care:
Home management has a ceiling. If three to four weeks of disciplined work hasn't moved the needle, or if the pain is sharp, deep, or radiating, it's time for a proper assessment. Running injuries almost never get better with rest alone, because the underlying mechanics are still there waiting when you start running again. Hip pain that has gone past the four-week mark deserves an assessment, not another rest week.
